USC Immunizations

 

**ALERT-- EFFECTIVE FALL SEMESTER 2007**
Meningitis vaccine is no longer recommended but REQUIRED for all first year students (entering freshmen 24 years of age and younger). If you decline a meningitis vaccine, your signature and date are required on a meningitis vaccine waiver form.  Parent signature required for students under 18 years of age.

The University of South Carolina requires ALL students born after December 31, 1956 to furnish the following proof of immunity in order to register for classes: 
 
A. MMR Vaccine   
     1.  Two MMR vaccines given on or after the first birthday
                 OR
     2.  Positive serum titer (blood antibody) to measles and rubella
                 
 
B. Meningitis Vaccine (all entering freshmen 24 years of age and younger).
    1.  Proof of Meningitis vaccine 
                 OR
    2.  Signed vaccine waiver form declining meningitis vaccine
 

IMPORTANT: Immunization records MUST be submitted to the immunization office prior to orientation in order to prevent any delays at orientation sessions and to be able to register for classes. 
 
Completed Immunization Forms should be returned by mail to Immunization Coordinator, Thomson Student Health Center, USC, Columbia, SC 29208 or faxed to 803-777-3955.
